To obtain a birth certificate, it must be the person themselves or parent only.  Must have valid picture ID.

If mailing in print application, fill it out and mail it in with a copy of driver's license or picture ID and $10 money order (No checks).

Application to Local Registrar for Copy of Birth Records (DOH-296A)

If applying in person come in to City hall, First Floor with some proof of identification.
